COVID-19 Vaccinations

Vaccine Record

St. Thomas Aquinas College will require all students and employees to become vaccinated so that this academic year will be as safe as possible and we can enjoy a “return to normal” campus experience. All vaccination records will be kept highly confidential.

Please submit your documentation via www.stac.edu/COVID no later than August 25, 2021 at 5:00 PM. 

  • Those who are fully vaccinated and have not already submitted their documentation, please visit www.stac.edu/COVID to submit proof of vaccination. By submitting this Form you are consenting to the College confirming your COVID-19 vaccination in the New York State Immunization Information System.
  • Those who will be requesting a medical or religious exemption request, please read our COVID-19 Vaccination Exemption Fact Sheet at www.stac.edu/exemption and fill out your applicable form. STAC students and employees exercising this right will be fully supported.


STAC International Students: if you have received a vaccine other than a United States Food and Drug Administration (FDA) or World Health Organization (WHO) emergency use approved vaccination, please contact STAC Health and Wellness Services at www.stac.edu/health for guidance.   

STAC employees: including student workers, are entitled to take up to four hours of paid leave each time they get a COVID-19 shot. At the moment, this means up to four hours for the single-dose vaccine and up to eight hours total for a two-dose vaccine.
