COVID-19 Vaccinations

Vaccine and Booster Records

St. Thomas Aquinas College requires all students and employees to become vaccinated so that our campus will be as safe as possible and we can enjoy a “return to normal” campus experience. All vaccination records will be kept highly confidential

Please submit your documentation via www.stac.edu/COVID

  • Those who are fully vaccinated and have not already submitted their documentation, submit proof of vaccination via www.stac.edu/COVIDBy submitting this Form you are consenting to the College confirming your COVID-19 vaccination in the New York State Immunization Information System.
  • It is the College's expectation that you receive the booster shot when eligible, as we believe the boosters will provide a critical additional layer of safety across our campus. When you are eligible to receive your booster, or if you already are, please get your booster and upload documentation at www.stac.edu/COVID.
  • Those who are requesting a medical or religious exemption, please read our COVID-19 Vaccination Exemption Fact Sheet at www.stac.edu/exemption and fill out your applicable form. STAC students and employees exercising this right will be fully supported.

STAC International Students: If you have received a vaccine other than a United States Food and Drug Administration (FDA) or World Health Organization (WHO) approved vaccination, please contact STAC Health and Wellness Services at www.stac.edu/health for guidance.   

STAC Employees: Those employed at the College, including student workers, are entitled to take up to four hours of paid leave each time they get a COVID-19 shot. At the moment, this means up to four hours for the single-dose vaccine and up to eight hours total for a two-dose vaccine.

Testing After Receiving Vaccine

Close Contact Quarantine After Receiving Booster

  • STAC employees and students who produce a valid “record of their booster vaccine” through the COVID-19 Vaccination Documentation Form will be exempt from quarantine if they are in close contact with a COVID-positive individual and asymptomatic.
  • Boosted individuals who are exempt from quarantine will still need to monitor symptoms and PCR test on day five following their exposure and wear a well-fitting mask until receiving the results of their PCR test.
  • If they do not have a valid booster record on file at STAC they will be required to follow the STAC Close Contact Quarantine process that is currently in place.
